Here are the steps you should take to report possible Medicare fraud:
- If you have questions or concerns about information on your Medicare Summary Notice or Explanation of Benefits, call your healthcare provider or plan provider first.
- If you are not comfortable calling your healthcare provider or plan provider or you are not satisfied with the response you get, call our toll-free hotline, or complete the form and someone from our office will contact you.
